Incident(s) Description:
In the course of investigating a report of hazing, it was determined that at a pass down event on 11/12/22 the following behaviors occurred: alcohol provided to underage members during a pass down event; alcohol consumed by underage members during a pass down event; high-risk consumption of alcohol competitions during a pass down event; physical activity; members forced to wear embarrassing costumes; members were invited to attend and did attend another event with alcohol prior to the pass down event; organization leadership and officers did not intervene to slow or limit the consumption of alcohol of new members.

Outcome(s):
Disciplinary Probation through Spring 2025, with once-a-month review meetings with a FSL staff member, headquarters staff member, and the President and/or designated executive team member(s) in attendance on chapter progress on stewarding chapter culture change and growth/maintenance of non-hazing new member practices

Ritual, Traditions, & the New Member Experience Education/Plan facilitated by Delta Sigma Phi Administrative Staff; with 100% chapter attendance; October 1, 2023

Sober Monitor/Health & Safety Education facilitated by JMU FSL staff; with 100% chapter attendance; December 16, 2023
