Lost your insurance card or need additional cards?

To request a new COVA Care or COVA HDHP (High Deductible Health Plan) ID card:

Step 1. To be mailed an ID card, your mailing address must be up-to-date in Cardinal. Visit Cardinal or contact your Benefits Specialist in the Human Resources office to be sure that your mailing address is correct.

Step 2. If your mailing address is correct: Call the ID Card Order Line at 1-866-587-6713. Provide either your health card ID number or your social security number. New ID cards will be sent to you within 10-14 business days.

Step 3. If your mailing address is incorrect: After taking the action in Step 1, wait 48 hours and follow Step 2.

To request a new COVA HealthAware ID card:

Step 1. To be mailed an ID card, your mailing address must be up to date in Cardinal. Visit Cardinal or contact your Benefits Specialist in the Human Resources office to be sure that your mailing address is correct.

Step 2. If your mailing address is correct: Call the Aetna Concierge Services at 1-855-414-1901 or online at www.aetna.com. Provide either your health card ID number or your social security number. New ID cards will be sent to you within 10-14 business days.

Step 3. If your mailing address is incorrect: After taking the action in Step 1, wait 48 hours and follow Step 2.
