CHBS is creating a portfolio of IPE and IDE learning activities and courses. Any faculty or a team teaching an interprofessional or interdisciplinary education learning activity can submit a request for that activity to be included in the Portfolio of IPE and IDE Learning Activities at CHBS.

SUBMIT INITIAL REQUEST FOR REGISTRATION

Once you submit the initial request for registration you will be contacted by the Associate Director for IPE and IDE to engage in consultation and complete the full form.

Why submit a request for registration?

• So that students who participate in IPE and IDE learning experiences can potentially be recognized at graduation.
• So we can identify IPE and IDE enthusiasts to further their engagement in faculty development and mentoring, and to form learning communities of practice.
• So the full extent of IPE and IDE learning experiences offered at CHBS is known.

How will the information provided in the requests for registration be considered?

A small team of peers, members of the IPE and IDE Council, will consider the request and determine the relevant categories for CHBS IPE and IDE student recognition initiative. This form/rubric will be used in the process.
